Life insurance provides financial protection for your family in the event of your death. Accidental Death & Dismemberment (AD&D) insurance provides additional coverage should you be seriously injured or should your death be a result of an accident.

In the event of an unfortunate situation, such as loss of life or serious injury, most families could not make ends meet without assistance from life insurance.

Life Insurance Benefit

Each school district offers its own life and disability benefits. Please contact your school’s HR/Benefits representative or visit your school’s benefits office to confirm your eligibility and plan options.

Accidental Death and Dismemberment (AD&D) Benefit

If death is a result of an accident, the beneficiary will receive an additional amount equal to your Life Insurance in force. If the employee is dismembered (such as loss of sight in an eye, loss of a hand, foot, or limb, hearing, speech, etc.), benefits will be paid to the employee as a percentage of the basic life amount.

Coverage Certificates

Life and disability insurance certificates provide a detailed summary of the benefits, provisions and coverage available under your plan. These documents outline eligibility requirements, benefit amounts, beneficiary information, limitations, exclusions and other important plan details.

Please contact your school’s HR/Benefits representative or visit your school’s benefits office to receive a copy of your coverage certificate.
